An important word about Protected Health Information (PHI) and your Connect Subscription

If you are subscribing to this service to receive health information about a loved one, by Law, the healthcare provider must have authorization signed by your loved one or their designated representative to give you (or anyone) protected health information either verbally or through the Connect service. Once the provider confirms that any required authorization is on file, they will activate your Connect subscription. You will receive an email notification when your subscription has been approved and activated.
